What you need to know

  • All travelers entering Tanzania through Zanzibar, regardless of their nationality, age or any other consideration, must carry their Tanzania Traveler Health Surveillance, completed online the 24 hours before arrival.

  • Children traveling with a guardian/parent, will also need to complete their own form.

  • It DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for your Visa when you enter the country. Keep in mind some nationalities will require a paper visa. If that is your case, we suggest you contact your local embassy.

Tanzania has opened up and relaxed COVID-19 regulations for tourists. Visitors no longer need a Health Surveillance form for mainland Tanzania. However, those planning to go to Zanzibar will still need to fill out the Tanzania Health Surveillance form.

The Health Surveillance Form will no longer be used by Tanzania as they reduce COVID-19 travel restrictions. However, the Revolutionary Government of Zanzibar revealed that it will continue to reinforce the implementation of COVID-19 mitigation measures to ensure the safety of everyone on the island.

With this in mind, registering to the health surveillance platform and filling out the form will remain mandatory for all visitors coming to the island. Here is everything you need to know about the Tanzanian health surveillance form and how to apply with iVisa.

What is the Tanzania Health Surveillance Form?

The Tanzania Traveler Health Surveillance is a screening document to monitor the health of travelers entering Zanzibar only. The form is part of Tanzania’s and Zanzibar’s health surveillance systems implemented during the COVID-19 pandemic. While you don’t need this form anymore to visit mainland Tanzania, it is still mandatory to submit it if you’re planning to go to Zanzibar.

Please note that it does not replace a visa. If your nationality requires a visa to enter Tanzania and Zanzibar, you will be required to present both the visa and the form upon arrival.

Who requires a Health Surveillance Form to enter Zanzibar?

All travelers, including tourists, children, returning residents, and crew members entering Zanzibar, both by air or sea, must fill out the Public Health Surveillance Form online. The form must be filled out by each member of a travel group.

How long is the Zanzibar Health Form valid for?

The form must be completed online at least 24 hours before your arrival to Zanzibar and is valid for a single entry and a stay of 30 days.

Requirements and documents needed for the Zanzibar Health Form

Although there are no document requirements for the Traveler's Surveillance Form, we will need you to provide the following information:

  • Your passport information, such as your passport number and expiry date.

  • Details of your trip including flight/boat times and dates.

Do I need a Health Surveillance Form if I’m transiting through Zanzibar?

Yes, all passengers transiting through Zanzibar are required to fill out a Tanzania Health Surveillance Form.

Documents and information required for the Tanzania Health Form

After approval, we will forward the completed Tanzania Traveler Health Surveillance with the Unique Health Code (UHC) to your email. It is recommended to print it out and hand-sign it. Please present it to immigration officers on arrival in Tanzania whenever requested.

Do minors need to fill out their own Health Form to visit Zanzibar?

Minors and children are also required to present a Tanzania Health Form of their own. The guardian or parent traveling with the minor is required to fill out the form on their behalf.

COVID-19 regulations and restrictions for Tanzania and Zanzibar

  • There are no longer COVID-19 test requirements.

Other documents you may require to enter Zanzibar

There are three possible visas that you can apply for to enter Tanzania and Zanzibar depending on your intentions to enter the country. They are:

  • Tanzania Tourist eVisa: The Tanzania e-visa allows citizens from eligible countries to travel to Tanzania and Zanzibar for tourism and leisure purposes, such as visiting family or friends.

  • Tanzania Transit Visa: A transit visa allows you to cross the country from one point of entry to another destination, i.e. enter Tanzania and simply pass through without exploring the country.

  • Tanzania Volunteer eVisa: In order to obtain a Tanzania Volunteer e-Visa, applicants must have a volunteering contract between them and the organization they will be volunteering for to prove their intent to volunteer. You may also need an invitation letter that confirms the purpose of the trip.

What else should I keep in mind before traveling to Tanzania/Zanzibar?

We recommend that all travelers to the island check updated information regarding COVID-19 protocols currently in place via government or airline sites, as these tend to change quickly.

Once you’ve checked the travel restrictions, got your visa sorted, and packed your bags, remember to apply for your Tanzania Health Surveillance form 24 hours before your flight.
